Field Trip Permission Copy

We are so excited to announce we are going to Medieval Times! May 24th is the Date.

The total cost is $35. Payment options available are Cash, Check (made out to SGC), or Credit/Debit Card. Please send Cash, Check, or confirmation of Online Payment with your PERMISSION SLIP before April 21. Online payments using Credit/Debit Cards should be made below with a service fee of 3.85%. You may pay in FULL or in 3 easy payments of $12.00.

A field trip permission form has come home with your child. PAYING and RETURNING a signed permission slip ensures your participation in this event! One doesn't work without the other!

*If you would like to add to our sponsorship fund to help another child go on this field trip, you can send money through any of the above options mentioned! Thank you.
